If you are new to our office please complete the Patient Information Packet (click here to download) and present them to our receptionist at the time of your appointment.

Please be sure that you also have the following available:

  • Your current health insurance card is required and will be photocopied at the time of your 1st visit. 

  • If your insurance requires a co-pay, this is expected at the time of checking in. 

  • Please bring any medical records to your appointment as well as any X-Ray films, MRI films, CT scans, or other studies that you have had recently. 

  • If a Referral Authorization is needed for your appointment, it is your responsibility to bring a written authorization with you. 

  • A copy of your state issued driver’s license or identification card.

  • If your claim will be filed under Worker's Compensation or 3rd Party Liability Insurance,you must provide us the following details:

1. Company's name & complete address
2. Your claim number 
3. Adjustor's name or contact person's name & phone number

  • If a patient is a minor (18 yrs. or younger), a parent or guardian must accompany the patient to their appointment, or the patient must have written consent from a parent or guardian in order to be seen by a physician.

If you have no insurance, or if this a liability claims case, payment in full is required at the time of service. If you are unable to make payment in full payment arrangements need to be made before the services are rendered.
